Takeda, Lundbeck, and Advocate Health Care (large US ACO), have collaborated to develop digital care pathway comprised of a patient-facing mobile health app, HCP-facing EHR integration and educational resources. The Pathway Platform is aimed at improving patient-provider engagement for patients diagnosed with major depressive disorder (MDD) and to demonstrate the value of an appropriate MDD clinical care pathway in a high impact health care system. The implementation of the Pathway Platform app in Advocate Health's PCP network increased patient-provider engagement during a MDD episode, demonstrating the feasibility and benefits of using a Mobile Health Platform for MDD management, measurement-based care (MBC), and shared decision-making (SDM) in patients with MDD and potential applicability in other conditions.
