CDTM expands the role of the clinical pharmacist in a team-based practice. Founded on published data demonstrating the significance of CP-based care on chronic disease management, CDTM in most states allows clinical pharmacist to prescribe for specific disease states to initiate, monitor, modify or discontinue drug therapy.


CDTM Clinic in our organization has been established for the clinical pharmacists to manage a variety of conditions including diabetes, hypertension, hyperlipidemia, behavioral health diseases, pain, thrombosis, and oncologic conditions. We have successfully incorporated CDTM into a portfolio of all that clinical pharmacists can do at top-of-license, while demonstrating significant contributions to ROI with a hybrid of services provided and clinical outcomes. 


Upon completion of this session, participants should be able to:


·        Describe clinical pharmacy clinics with CDTM

·        Explain how CDTM helps close care gaps for patients with chronic diseases

·        Justify the benefit to key stakeholders in development of a clinical pharmacy program by examining overall ROI and clinical outcomes
