Join this webinar for an introduction to the Rheumatoid Practice Improvement (RAPP) Project strategies for improving treatment capacity. Many RAPP physicians, practice teams and most importantly patients report being enthusiastic about the benefits that population management provides for care and disease outcomes.
· Using RAPP population health strategies, practices can gain a complete picture of their RA patient population (e.g. number of patients with RA in their practice, status of Disease Activity Assessments)
· Practices that have implemented these strategies note that it requires the adoption of better-defined systems of care that include:
o Selecting Disease Activity Assessment measures
o Implementing a system to collect data
o Using a population registry - which can be as simple as an Excel spreadsheet
o Managing and prioritizing patient visits based on the results of the registry to have each team member work to his or her highest degree of licensure and skill
o Overall, the RAPP Project may help improve the health of the RA population by ensuring that appropriate care is provided for those patients who need it the most
