To build and defend patient relationships and revenue during the pandemic, many value-based provider organizations have created virtual care strategies that include telehealth and remote patient monitoring. Such technology provides opportunities to engage patients in the home and to support chronic condition management, yet high-tech, device-based remote patient monitoring is limited in its scale and its resulting impact due to high cost and limited accessibility.

Esse Health prioritized a scalable approach, utilizing deviceless remote patient monitoring for over 1,000 Medicare Advantage patients with chronic conditions including heart failure, diabetes, and COPD. With technology, the care management team was able to provide proactive care to an important population, the rising-risk, resulting in measurable improvements to clinical and financial outcomes, including a 46% drop in heart failure ED visits, and a $250 PMPM savings – a 19% reduction in global costs.


This session will cover the processes and successes of implementing deviceless remote patient monitoring in a Medicare Advantage population. Lessons learned will be directed to senior leaders in population health, virtual care, and value-based care operations.
