Value-based healthcare encourages primary care providers to refocus on the fundamentals of good health, with diet and lifestyle at the forefront. Poor nutrition is a common occurrence in the outpatient setting in patients with chronic conditions as well as those recently discharged from the hospital. Up to 1 in 3 older adults is either at risk of becoming or is already is poorly nourished.


Nutritional interventions, including the use of oral nutritional supplements and/or meal replacements, is a low-cost, low-risk solution clinically proven to improve patient outcomes and reduce healthcare costs, yet is rarely incorporated into patient-centered care to achieve these goals.


This webinar will demonstrate how a large hospital system and a primary care practice adopted a nutrition quality improvement program consisting of early identification of nutrition risk, intervention and education to improve patient outcomes and reduce healthcare costs.
